Additionally, review our financial policy. If you are seeking Medication Management, please also fill out the Request Med Management form located in the top menu!
If your clinician accepts insurance, please take a picture of the front and back of your insurance card and email it to: assistant@knoxbehavioral.com. This will help expedite your mental health evaluation and any necessary therapy services. (Note: We can only process insurance cards received via email.) Additionally, don’t forget to complete the new patient referral form for a smoother intake process.
